Amenorrhea
Secondary amenorrhea, dysmenorrheaprimary, amenorrhea, menorrhagia
Amenorrhea is the term used to describe the abnormal absence of menstruation
for a long duration of time. There are two types of amenorrhea, primary
and secondary, primary amenorrhea is a condition when menstrual cycle
has not begun in a girl till the age of 16, and secondary amenorrhea is
a condition where the menstrual cycle abruptly stops and does not take
place for duration of three months or more. The menstrual cycle may be
affected due to several factors; this includes stress, sudden illness
or hormonal imbalance.
Pregnancy is a common cause of amenorrhea in young women, this generally
happens when the pregnancy is undetected. Other than this, extreme forms
of exercise, hypothalamic problems and anorexia may also lead to amenorrhea.
The main cause of primary amenorrhea is attributed to genetics or abnormal
anatomic condition in young women. This may happen when the reproductive
organs have not developed properly during the fetal stage itself and the
problem exists due to birth defect. Causes for secondary amenorrhea are
attributed to some disorder in ovaries, pituitary gland, hypothalamus
and uterus.
Amenorrhea is diagnosed by examining the medical history of the patient.
This is scanned for preexisting conditions related to the reproductive
system and medications being taken, if any. Other than this physical examination
of pelvic area may also be performed. Once the medical history and physical
examination has been performed, blood and other tests such as ultrasound,
CT or MRI scan, and X-ray may be recommended to detect hormonal or other
physical changes.
The treatment methods employed depends on the cause of amenorrhea. The
aim of the treatment includes establishing normal menstrual cycle, relieve
symptoms of hormonal changes through hormone therapy and prevent complications
related to amenorrhea. In case of genetic or anatomical causes, surgery
may be considered as an option to correct the condition. Other causes
such as emotional stress, extreme physical stress, weight problems and
illness can be treated with normal treatments available for the cause.
The main complication that may develop due to amenorrhea is infertility;
a woman detected with this condition would be unable to have successful
pregnancy unless treated for the cause of amenorrhea. Prolonged amenorrhea
may also lead to osteoporosis especially in older women, the reason being
depletion in levels of estrogen leading to changes in bone density. Amenorrhea
can be prevented in case the underlying cause is excessive physical or
emotional stress and weight gain. In cases where the cause can be addressed
through medication, normal menstrual cycle may begin once the treatment
is over.
